When visiting one of our Patient Service Center's, please bring the following information with you:
- IRL test requisition form/script from your health care professional requesting laboratory testing
- Current insurance identification card (Medicare, HMO/PPO/EPO, Private Insurance…)
- Photo ID (e.g. driver’s license, employee identification)
Minors must be accompanied by their parent or legal guardian.
If you are bringing a minor to one of our Patient Service Center's and you are not the legal guardian, you will need to bring a notarized statement signed by a parent or legal guardian authorizing you to give permission for the collection/billing of the tests ordered by the minor’s physician.

Do I need to make an appointment to have my blood drawn at a Patient Service Center?
At IRL, appointments are not necessary. We offer short wait times and a quick registration process. Bringing a completed IRL test requisition form or a prescription from your physician, a copy of your medical insurance card and a photo ID will speed the registration process. Our patient service centers are usually busier in the early morning when patients whose testing must be drawn “fasting” arrive. Not fasting? Consider coming in after 10 a.m.

Before having my specimen collected at a Patient Service Center, do I need to fast?
Many lab tests are more accurate when performed on a fasting blood sample. Your physician will tell you if (and for how long) you should fast before having your blood drawn.
Can I request my own laboratory tests?
Patients cannot order laboratory tests themselves. If you are interested in having a specific lab test performed, please be sure to talk to your physician.
How do I obtain my lab results?
At IRL, your lab tests are reported directly to your physician/healthcare provider. Federal and State laws require us to send your results only to your ordering physician. Be sure to ask your physician to contact you with your test results.
What types of laboratory tests does IRL perform?
As a clinical laboratory, IRL is licensed and certified by the state and federal governments to perform tests on blood, body fluids, tissue and other materials to diagnose and treat diseases of humans. The IRL menu includes tests for allergy, anemia, cancer, diabetes, and diseases of the kidney, liver, prostate, thyroid, and digestive tract. In addition, our state-of-the-art Microbiology department offers testing in bacteriology, virology, mycology, mycobacteriology, parasitology and molecular diagnostics.
